In preparation for our session, here are all the details you need ↓ 


 

1. Complete Sleep Assessment Form

Please complete the Sleep Assessment Preparation (SAP) form by clicking on the button below. It will take you less than 10 minutes to complete but will save us time in your assessment. I will review your answers before you sleep assessment so please make sure you submit this form at least 24 hours before your appointment time.

2. Join Meeting On Zoom

The session takes place online via Zoom.

  • Please make sure you are able to access Zoom on the device you would like to use.
  • You will find the link to your Zoom call in the booking email you have received (see image below).
  • Please make sure you have a quiet place to do the session where you are not distracted and are able to talk freely.
  • Please have your camera and audio on
  • When it is time for your assessment, just click on that link and it will take you straight to our meeting.
